Maternal and Infant Health

Description
The group undertakes studies on research questions that impact upon public health services and on clinical practice in maternal and child health, emphasising high quality designs.Current and recent randomised controlled trials (RCTs) generate maternity care evidence to answer such questions as: can a peer support service increase breastfeeding; can a pregnancy outreach worker service improve antenatal care engagement, psychological health and infant outcomes of ethnically diverse women with high social risk.In intrapartum care, RCTs to investigate effects of a range of clinical practice modifications to increase spontaneous vaginal births include: types of epidurals, high or low dose oxytocin for delay in labour and maternal position in second stage labour with epidural.Re-designed postnatal care to manage health problems and exercise to reduce postnatal depression are examples of recent and current RCTs within postpartum care. Childbirth-related health has long been a focus, with observational studies to generate hypotheses that are subsequently tested in RCTs. For example a 12 year cohort study on postpartum urinary and faecal incontinence led to trials initially in pelvic floor muscle exercise for symptomatic women and recently to prevent uterine prolapse.Systematic reviews on, for example peer support to increase breastfeeding, exercise for postnatal depression, antibiotics for preterm rupture of membranes and treatments to reduce obesity in pregnancy are undertaken.Studies on perinatal and maternal mortality and morbidity in developing countries, for example maternity waiting areas, maternity care strategies involving traditional birth attendants are pursued.

Medawar Centre for Healthy Ageing Research

Description
We are an ageing population, with current demographic trends indicating that 1 in 5 adults in the UK will be aged over 65 by the year 2020. Whilst this is a cause for celebration, there is also evidence that healthspan (the time spent in good health) is not keeping pace with the increases in average lifespan, with significant consequences for quality of life in old age and for health and social services provision.

Mental Health

Description
This group is multidisciplinary in nature, focuses mainly on intervention development and evaluation, but includes epidemiological and policy work.The mental health group has been in existence since 2001. In particular, we aim to develop and test pragmatic interventions for use in primary care, but we are also engaged in explanatory trials and translational research. We have a particular focus on the care of people with serious mental illness including the commissioning, development and evaluation of early intervention services, service users' views of care and improving primary care mental health.Current work includes:modeling a framework for how mental health, substance misuse and learning disabilities services could be commissioned by Local Authority and NHS commissioners a cochrane review of collaborative care for people with serious mental illness and cross sectional study of current care for people with serious mental illness exploring service user, carer and providers' views of services for people at the point of discharge from Early Intervention Services a trial of bespoke smoking cessation therapy for people with schizophrenia qualitative evaluations within trials of CBT for command hallucinations, computerised CBT for depression and Joint Crisis Plans to reduce compulsory treatment of people with psychosis.
