Improving access to psychological therapies for people with long-term conditions

Service evaluation of a project to improve access to psychological therapies for virtual ward patients.

This six month project seeks to embed psychological therapists into a multi-disciplinary enhanced assessment team supporting both the physical and psychological reablement of patients with long-term conditions and other physical health problems/disabilities.

This is based on the evidence regarding the prevalence of co-morbid mental health problems in cardiovascular disease, diabetes, COPD and chronic musculoskeltal disorders and the local experience that such patients are not accessing psychological support.

HSMC will undertake an evaluation of the impact of the pilot and the processes that have led to these impacts.
