The Midlands is leading the way on Population Health Management!

Over the last year Midlands STP/ICS have participated in the PHM Academy. This has been the first regional programme designed to support STPs/ICS’ to embed Population health management (PHM) as an approach. It included a multidisciplinary team working through the PHM cycle on a chosen project area, and sessions for analysts on the technical skills required for PHM.

This full day event is designed to share the learning from the Midlands Academy, celebrate what teams have achieved and think about the next steps for the spread of PHM.

It is aimed at everyone - within the region and beyond - who wants to learn from what has been achieved in the Midlands and improve their practice on PHM.

Population health management (PHM) is essential to integrated care. It provides health and care systems with an evidence-based means of achieving better population outcomes within the resources available. PHM’s basic theory is that: better insight = better decisions; and that better decisions = better outcomes.
