The New Application form is for projects intending to produce discovery results, and/or which require >5 donors, and/or which require clinical data to be associated with samples.
The Method Validation form is for projects intending to use a small amount of readily-available material as technique controls, or to establish the basic validity of a technique, using material from 5 donors or fewer, with no clinical data permitted beyond a confirmation that samples meet the selection criteria.
The Application Amendment form is to request an amendment to either a “full” application or a method validation, once initial approval has been received.
The Application Extension form is to request an extension of the length of a project's approval.
